The Importance of Cancer Fight in Caucasus

According to WHO last data, 10. 000. 000 persons are getting cancer and 6. 000. 000 are dying annually in the world at the present time. About year 2020, the cancer incidence will raise up to 20. 000. 000, and the mortality - 10.000. 000 cases. Finally, the issue of cancer will pass the borders of health care and gain the significance of global social problem. That's why, the cancer fight already represents the prioritized direction of social sphere.
It also should be mentioned, that person gets cancer in the time of professional and creative development (45-65 year), when one's activity brings the maximal effect. That is the reason, that cancer causes the huge economical damage to the world and obstacles it's development.
Most of the world scientists, the leader specialists of American Cancer Society (ACS, 2001) and International Union Against Cancer (UICC, 2001) among of them, came at final point, that the special attention of cancer fight should be stressed on health promotion and diseases prevention, which represents the prioritized direction of XXI century.
According to American Cancer Society (ACS, 2001) and International Union Against Cancer (UICC, 2001), prevention gives the possibility to reduce the development of 70% of all sites cancer, 35% of them are in cause-result correlation with alcohol and tobacco abuse and 35% - with incorrect diet. Due to life style changing the development of 90% of tumors could be avoided.
Caucasus, which geographically consists the regions south and north beyond the Caucasian Crest and is situated between the Black and Caspian Seas, represents the unique entire ecological body. The ecosystems of Caucasian countries are significantly interacted and dependent on each other. Evidently, administrative borders couldn't divide them. Naturally, and more frequently anthropologically derived changes in one of the Caucasian countries ecosystems are effecting the population of this country and ecosystems and population of neighboring countries as well. To avoid the possible anthropologically derived cataclysms and associated severe results the overall readiness and establishing of environment epidemiological control are especially significant at the present time. Consequently, every Caucasian country and their entire population are interested on working out of global Caucasian environment protection policy and hygienic standards. The last represents the prioritized direction of cancer fight in terms of cancer prevention issues. Considering the interstate trading and bartering of goods, the working out of entire policy and hygienic standards in field of manufacturing and trading of dietary and everyday-use products is very important.
Starting of cancer primary prevention in Caucasus will be possible after the realization of following goals:
1. Protection of environment in Caucasian countries (atmospheric and working place air, soil, drinking water, Black and Caspian Seas, flora and fauna, dietary products, products of massive use, and etc) from pollution by chemical and radioactive carcinogens and biological agents (viruses, bacteria and etc), reglamentation of carcinogenic agents exposure, population health promotion and cancer prevention;
2. Popularization of healthy life style in Caucasian countries first of all should be directed against the tobacco smoking;
3. Preparation of legislative packages and advocacy efforts for their adoption by Caucasian countries parliaments, which will prohibit:
  • Tobacco smoking in governmental and non-governmental organizations and places of public meetings;
  • Tobacco products (as one of the mean of health hazardous narcotics) and strong alcoholic drinks advertisement;
  • Advertisement of dietary products, non-alcoholic drinks and other products of massive use, without the preliminary testing on content of carcinogenic agents, and consequently without appropriate certificate.
    According to decision of I-St congress of Trans-Caucasian Oncologists (Tbilisi, 1998), in order of Caucasian population social protection and coordination of cancer fight activities, the following tasks were determined:
  • Creation of entire cancer fight information- and communication system between Caucasian countries (Tbilisi-Baki-Erevan);
  • Working out of global regional policy and creation of Caucasian program of cancer fight, with preparation of appropriate inter-country legislation acts;
  • Preparation of joint projects for granting on prioritized issues of cancer fight.
    At the present time, because of economical situation in region, where the adequate financing of cancer fight programs is not possible, for solving of prioritized goals of region, I-St congress of Trans-Caucasian Oncologists first of all decided the creation of non-governmental societies and unions.
    Besides, at the present time of cultural and economical globalization process, when the Caucasian countries and region international intersectional integration becomes inevitable, the establishing of Caucasian countries non-governmental professional relations with their future development, also taking of joint efforts for cancer fight intensification and putting them to new steps of progress gains drastic importance. The creation of Caucasian professional union will promote every above-mentioned issue.
    All above-mentioned verifies the necessity of creation of Caucasian Union Against Cancer (UCCC).
    For raise of effectiveness of cancer fight in Caucasus, the advantage of creation namely non-governmental union is inevitable, because it's activity will be concerned with preparation of normative and legislation acts, followed by their lobby in governmental structures.

    Paragraph 1. GENERAL REGULATIONS

    1.1. Caucasian Union Against Cancer (UCCC) is a public non-government, non-religious, non-profit, non-political organization, which is established by the voluntary unification of organizations and individuals with various professions and profiles. The main aim of the Association is to stand up for social and public health defense of population in Georgia (Caucasus region), cancer prevention, to help prolong the average expecting life, to integrate country (Caucasus region) in international structures and to speed up economic development.
    1.2. Due to realization of created Conception: "National Model of Cancer Prevention" Union will serve for cancer prevention in Caucasus, reglamentation of population exposure to carcinogenic agents, preparation of legislative basis and protection for environment, improvement of medical service and insurance system by fundraising efforts, implementation of new technologies and technical means, also systems of distant learning and diagnosis due to organization of Tele-medical consultations and Tele-conferences.
    1.3. The aims of the Union are concerned with working out of policy, strategy and tactic of scientific- and practical activities of cancer fight, which considers the organizational, methodological, scientific, preventive, educational, editorial, projective- and constructive, legislative and mediator activity.
    1.4. Besides oncologists, the members of the Union are: general profile doctors, epidemiologists, cancer registries, radiologists, ecologists, sanitary-hygienists, veterinarians, biologists, chemists, physicists, statisticians, computer programmers, lawyers, economists, businessmen, journalists, TV operators, other non-government organizations and individuals with different professions.
    1.5. The Tbilisi Head Office conducts its activities in accordance with the Constitution of Georgia, "Citizens Public Unions Laws", the Constitution of the Union and other laws currently in force. Baki and Erevan offices (when they are open) conduct their activities in accordance with the laws and legislations of their countries
    1.6. The Union is non-government, non-political, non-profit juridical structure possessing private property, independent balance, monetary and financial settlements in credit-and-banking institutions, round stamp, emblem and other attributes.
    1.7. On behalf of its name, the Union may acquire propertied and personal non-propertied rights and take obligations, be plaintiff or defendant in a court or arbitration tribunal.
    1.8. The Union is responsible for its obligations by owned property. It doesn't take responsibilities for state or government obligations, as well as the state or the government is not responsible for Union obligations.
    1.9. The Union is empowered to act according to the Statutes.

    Paragraph 3. UCCC AIMS AND GOALS

    3.1. UCCC Aims:
    3.1.1. The main aim of the Union is to stand up for social and public health defense of population in Caucasus, cancer prevention, to help prolong the average life, to integrate region in international structures, to speed up economic development and promote the creation of open society,
    3.1.2. Due to realization of created Conception: "National Model of Cancer Prevention" Union will serve for cancer prevention in Caucasus, reglamentation of population exposure to carcinogenic agents, preparation of legislative basis and protection for environment, improvement of medical service and insurance system by fundraising efforts, implementation of new technologies and technical means, also systems of distant learning and diagnosis due to organization of Tele-medical consultations and Tele-conferences.
    3.1.3. The main aim of the Union is to determine strategy, tactics and politics of scientific-practical activity in the field of cancer control. It means organizational-methodological, scientific, preventive educational-teaching, propaganda-publishing, projective-constructive, legislative activities.
    3.2.UCCC Goals:
    3.2.1. Organizational goals:
  • To organize representative offices in Baki and Erevan,
  • To create the UCCC Web Site,
  • To create Caucasian Network of Cancer Control (CNCC: Tbilisi-Erevan-Baki)
  • To create Caucasian Network of Cancer Registries (CNCR: Tbilisi-Erevan-Baki)
  • To create Tele-morphology system: Tbilisi-Batumi
  • To create Tele-medicine system: Tbilisi-Erevan-Baki
  • To create Caucasian Tele-medical Consultation Board (CTC)
    3.2.2. Methodological goals:
  • To determine the common policy and strategy of Cancer Control in Caucasus region.
  • To work out the Caucasian Plan of Cancer Control.
  • To work out the common sanitary-hygienic standards in Caucasus region (to determine the maximally permissible content of tar and nicotine in tobacco production and dosage and concentration of each carcinogenic substances in environment).
  • To work out Cancer Notification Form according to recommendations of the International Association of Cancer Registries and to adopt it at the meeting of the Caucasian Association of Cancer Control.
    3.2.3. Scientific goals:
  • To determine priorities of scientific-research work.
  • To help coordinate scientific-research work.
  • To work out and adopt new methods, methodologies and technologies.
  • To finance preferable scientific works of the Union and its individual members on the basis of submitting written request and Union Expert decision.
  • To organize cooperated investigations (including international) according to programs and projects, to take orders for conducting scientific-research work based on contracts from different funds (as a grant), government and private structures.
  • To organize conferences, seminars and other scientific activities. To take part in analogous activities in foreign countries.
  • To organize scientific competitions and award the winners with prizes and other forms of rewards.
  • To conduct scientific competitions and to grants the winner projects.
  • To start creating the computer dBase of cancer incidence in Caucasus according to Cancer Notification Form, to process the dBase statistically, to prognosticate cancer incidence, to create and publish epidemiological maps of cancer incidence in Georgia (Caucasus) according to descriptive epidemiological studies.
  • To conduct cancer epidemiological research in Caucasus.
  • To evaluate the efficiency of cancer control programs in Caucasian countries.
    3.2.4. Medical Goals:
  • To prepare and realize cancer screening and early detection programs in Caucasian countries
  • To coordinate Tele-medicine system Tbilisi-Erevan-Baki;
  • To coordinate Caucasian Tele-medical consultation board;
  • To organize distant screening of cancer in Caucasian countries and to evaluate its efficiency using Tele-medicine;
  • To evaluate and carry out monitory control of distant results of cancer treatment in Caucasian countries;
  • To help improve the efficiency of oncology patients medical service and to organize consultations with world-leader specialists;
  • To introduce new methods and technologies;
  • To help create the laboratories for determination the far and nicotine content in tobacco products;
  • To help create the laboratory for determination the dosage and concentration of chemical and radioactive carcinogenic substances in atmosphere and working place environment, soil, drinking water, alcoholic and non-alcoholic beverages, food and etc.
    3.2.5. Educational Goals:
  • To work out the educational programs for training the specialists in the field of cancer control.
  • To establish permanent advanced training courses in the field of cancer control (including joint courses together with educational institutions of foreign countries).
  • To prepare specialists in the field of cancer control at the different stages of their Pre-and-Post graduate training (residency, qualification courses, training at the work place, etc).
  • To set up special scientific council (in the field of cancer control), which will conduct certification of members according to the regulations adopted by the Union.
  • To conduct licensing and certification of Union members, to give them appropriate certificates.
  • To promote the training of scientific staff (post-graduate courses, conferences, symposiums, congresses, seminars and other scientific forums).
    3.2.6. Propagandistic and Publishing Goals:
  • To place Caucasian Program of Cancer Control on Union's Home Page.
  • To place Epidemiological Maps of cancer incidence in Caucasian countries on Union's Home Page.
  • To rise the level of hygienic-sanitary knowledge of population by promoting healthy life style using TV, radio, press, etc.
  • To publish 10 modified recommendations concerning healthy life style and distribute booklets among the population in order to reduce cancer morbidity.
  • To publish monographs, textbooks, epidemiological maps, atlases, brochures, reports.
  • To publish magazine: "Cancer Epidemiology and Control in Caucasian Countries".
  • To conduct advertising activity.
  • To control advertising in Mass media.
  • To popularize scientific achievements in oncology.
  • To review inventions.
  • To finance the publishing of important scientific works.
  • To conduct the lectures, seminars and training at work place.
    3.2.7. Projective-and-Constructive Goals:
  • To plan the development of cancer services in different regions of Caucasus;
  • To help reconstruct and build cancer control offices;
  • To conduct scientific-research, experimental-constructive, projective and service work in a field of medicine.
    3.2.8. Legislative Goals:
  • To lobby the obligatory submission of Cancer Notification Form in the Health Care System of Caucasian Countries (Georgia, Armenia, Azerbaijan);
  • To lobby the establishment of common hygienic-sanitary standards (standard of tar and nicotine permissible dosage and concentration of each carcinogenic substances in environment) in the Health Care System of Caucasian Countries (Georgia, Armenia, Azerbaijan);
  • To prepare and lobby the bill in the Parliament of Caucasian countries, which will protect the environment (atmosphere, work place environment, Black Sea, flora, food) from the pollution by chemical and radioactive carcinogenic substances, biological agents (virus, bacteria, etc), put limit on their effect on population, set testing and certification on cancer risk for national, exported, imported products (food, tobacco product, alcoholic and non-alcoholic beverages), set up fines on cancer risk products;
  • To prepare and lobby the bill in the Parliament of Caucasian countries which will prohibit:
    1. Tobacco smoking in state and non-state organizations and public places;
    2. Advertising tobacco products (as a harmful narcotic substance for human organism), strong alcoholic beverages;
  • To prepare and lobby the bill in the Parliament of Caucasian countries which will prohibit advertising food, non-alcoholic beverages and other products without their testing on the content of carcinogenic agents;
  • To prepare and lobby the bill in the Parliament of Caucasian countries which will put Union control on advertising food, non-alcoholic beverages and other products.

    Paragraph 4. UCCC MEMBERS RIGHTS AND OBLIGATIONS

    4.1. Individuals as well as collective group may become Union member;
    4.2. Individual applicant could become the Active and Honorary members;
    4.3. The Union member may become an individual who will recognize the Union Constitution and will honestly carry out membership obligations;
    4.4. The main requirement for Active membership is high professionalism and sense of responsibility of the person. Nationality, religious affiliation, political orientation, sex, age, marital status will not be taken into the consideration;
    4.5. Individuals who want to participate in the Union programs (Union member who will get paid according to Union contract) must get interviewed and tested on professionalism. Selected members will sign the contract;
    4.6. The main requirement for Honorary membership is high professionalism and level of contribution in field development.
    4.7. Work enterprises, organizations, institutions, firms and companies (despite amount of the property), Caucasian and foreign public unions, scientific associations and societies, funds who recognize the Union Constitution and pay registration and annual fees may become Union Associate members.
    4.8. Individual citizens, their unions and work groups may become association members by submitting written request to the Union Board.
    4.9. The Union individual member can leave the Union by submitting written request to the Board of Administration. The Board of Administration must summarize and make decision no later than one month after getting the request form.
    4.10. The Union assosiate member can leave the Union by submitting written request to the Board of Administration. The Board of Administration must summarize and make decision no later than one month after getting the request form.
    4.11. The Board of Administration may include or discontinue membership of member by open ballot and simple majority of votes.
    4.12. Founders of the Union who have taken part in the work of the Constituent Assembly may become Union members beginning from the state registration of the Constitution.
    4.13. The Union member has right:
  • to discuss the problems concerning the Union activities; to take personal part in working out and realization of plans, projects, programs of the Union;
  • To select and be selected in the Union Administration
  • To get information about Union work;
  • To use the privileges and financial assistance prescribed by the Union;
  • To help the Union financially and materially;
  • To ask the Union to help in securing their legal interests belonging to the Union regulatory activities;
  • To be member of other public unions;
    4.14. Obligations of the Union members are:
  • To take active part in the Union activities, to show initiative, to help successful realization of Union tasks, to make propaganda of Union goals and tasks;
    4.15. The Union members are obliged to pay registration and annual fees.
    4.16. The Union Administration makes registration and annual fees for individual and collective members (different for Caucasus and foreign countries), economic plans and annual budget of the Union.
    4.17. Union member, who doesn't honestly carry out membership obligations, damages the reputation of the Union or violates the Statutes or the Rules may be excluded from Union membership.

    Paragraph 5. UCCC RIGHTS

    5.1. According to Constitution, the Union has right: UCCC RIGHTS To own buildings, transportation and communication equipment, pecuniary funds, stocks, other valuable papers and property, which is necessary for securing the Union activity;
  • To work out and accomplish the Union plans;
  • To introduce and protect Union members interests in the state organs;
  • To make contracts and agreements with Caucasus and foreign firms, organizations, institutions, private individuals;
  • To establish local and regional organs, enterprises. representatives, other public unions;
  • To conduct publishing activity and have periodical publishing organ;
    5.2. The Union has right to rend property as well as lend, sell or give for a grant its own property;
  • The Union has right to accomplish industrial and economic activities for realization of the Untion tasks;
  • The Union has right to accomplish other activities not contradicted with Caucasian countries legislation and the Union Constitution.

    Paragraph 6. UCCC ORGANIZATIONAL STRUCTURE

    6.1. The highest organ of the Union is the Union Members Joint Meeting, which is held once in four years. Advisory-Consulting Board, Union administration and Inspection Committee have right to call non-planed joint members meeting.
    6.2. The Union Members Joint Meeting is considered authorized if 2/3 of Union members is presented. in order to be valid, decisions of the Joint Meeting require a simple majority of votes.
    6.3. The Rights of the Union Members Joint Meeting are:
  • To determine priorities of the Union activities;
  • To adopt strategic programs, projects and plans of the Union;
  • To listen and confirm reports of the Board of Administration and Advisory-Consulting Board to elect the Executive Board of the Union;
  • To make changes and addenda in the Union Constitution and inform the registration organ about these changes;
  • To solve other important aspects of the Union activity;
    6.4. Within the period between the Union Members Joint Meetings the Union activity will be conducted by the Board of Administration. The President, the Vice-President, the leaders of association subdivisions and the Union Secretary constitute the Union Administration, which is empowered to rule over the Union activity between the Union Joint Meetings periods;
    6.5. The President of the Union
  • Is initially elected at the Constituent Assembly by the initiative group. The President is reelected at the Joint Meeting of the Executive Board and Advisory-Consulting Board;
  • Presides administration meetings with the right of casting vote;
  • Makes permanent and temporary subdivisions, according to the Union activity;
  • Appoints Union employees, including association secretary;
  • Forms the available staff and numerical strength of the Executive Board;
  • Manages the Union property and funds. On behalf of the Union, the President concludes contracts;
  • Represents the Union interests in Caucasian states authority and ruling organs, economic and public organizations, at the international forums and in other organizations;
  • Makes individual decisions, which will be discussed at the next meeting of the Executive Board, if necessary; Issues orders and instructions within the limits of his competence.
    6.6. The Vice-President of the Union:
  • Is elected from the staff of the Executive Board at the Executive Board Meeting;
  • Takes part in the Executive Board Meetings with the right of casting vote;
  • Exercises control on the fulfillment the orders and instructions, recommendations of Union Members Joint Meeting, the Executive Board and President;
  • Acts on behalf of the Union name;
  • Undertakes the President's obligations in a case of his absence.
    6.7. Rights of Union Board are:
  • To consult with the president about the strategic aspects of the Union;
  • To give recommendations to the Executive Board in order to optimize of the Union activities;
  • To control methodological work of the President and the Executive Board;
  • To take part in the Executive Board Meetings with the status of honorary member and the right of casting vote.
    6.8. The Board meetings will be held in a case of need but no later than once in a quarter.
    6.9. The President of the Union and the vice-president call the Board Meetings. In order to be valid, decisions of the Executive Board Meeting require a simple majority.
    6.10. The Board is responsible for administration work to Union Members Joint Meeting.
    6.11. The extended representation of the board will be organized by board itself and representative offices executive directors.
    6.12. The extended representation of the board meets once in a four year to prepare changes and addenda of Union Constitution for submitting for consideration at the Union Members Joint Meeting;
    6.13. The Union representative offices are headed by executive directors, who are initially elected by board according to presentations from appropriate countries leader specialists. They will be re-elected on joint members meeting.
  • The executive director sets the permanent and temporary subdivisions in his countries,
  • The executive director appoints employees of the office,
  • The executive director Sets salaries and other forms of financial stimulation for association employees;
  • The executive director transfers the membership fees on a bank account of the country,
  • The executive director manages office property and funds. On behalf of the office, makes contracts,
  • The executive director exercises control on the fulfillment the orders and instructions, recommendations of Union members joint meeting, Union board and president,
  • The executive director takes part at the extended meeting of the Union board with the right of decisive voice,
  • The executive director acts on behalf of the Union board name,
  • The executive director gets into contacts with analogous foreign organizations, firms and private persons;
    6.14. Machinery of the Union is formed by each office of the Union. It consists of temporary or permanent departments, laboratories and other structural subdivisions. The Union employees receive salaries according to individual contracts.
    6.15. The Association Secretary is appointed by president. Union secretary coordinates the Union plans and programs, prepares agenda and other necessary documents for the Executive Board Meetings, takes secretary responsibilities at these meetings, prepares orders, instructions, projects and other documents for the President to sign.
    6.16. The Inspection committee is initially elected at the Constituent Assembly for four years period and with three-member constitution (one from each country). The committee is reelected at the Association Members Joint Meeting.
    It's functions are as follows:
  • To control functional-economic activities of the Union and its enterprises;
  • Member of committeeecan not at the same time be a member of the Union Administration;
  • Committee members may participate in the work of the Board Meetings with the right of consultative vote.

    Paragraph 7. UCCC REPRESENTATIONS AND OFFICES

    7.1. Union offices are at first established in Tbilisi, and only after that in Baki and Erevan. In a future, representative offices can be established in any capital city of Caucasus countries, as well as in any town of any country,
    7.2. Tbilisi office is a head office in Caucasus region. Other representative offices are head offices in their countries and they can create branches throughout their countries,
    7.3 After registration in their countries, Baki and Erevan offices can have their own independent bank account and round stamp,
    7.4. Each office of the Union can independently establish preventive and rehabilitation centers, consiliums, information medical systems and nets, laboratories, auxiliary enterprises, and other branch offices.

    Paragraph 8. UCCC PROPERTY AND INCOME

    8.1. General funds, circulation capital and other property, which are shown in independent balance make up the Union property. This property is used for maintenance the Union activity.
    8.2. Union income constitutes:
  • Registration and annual fees from individual and collective members of the Union ,
  • Charities from Caucasian and foreign juridical structures;
  • The Union gets grants and donations from different funds, charity organizations, state and public organizations, enterprises and private persons;
  • Granted and inherited income;
  • Income from publishing work;
  • Income from own enterprises;
  • Other ways of income which doesn't contradict with Georgian legislation.
    8.3. Union self-financing plan In case of adoption a bill by the Parliament the Union plans to go on to the self-financing using the following sources:
  • To pay cancer risk fines to Union (according to Parliament decision):
  • To pay tax to Union for the control of testing the advertised products (according to the Parliament decision);
  • To pay tax to Union for international councils and Tele-medicine services.
    8.4. Part of the each Union office income is used as a payment to employees and the rest for the Union further development.
    8.5. Income from enterprises is used for accomplishment goals and tasks of the Union.
    8.6. Information about Union work will be placed on the Union Home Page and spread via the Internet, which will help to put the Union activity on monitory control.

    Paragraph 9. UCCC INTERNATIONAL CONTACTS

    9.1. The Union has right to unite with analogous international organizations and unions as a collective member, to establish direct international contacts, to take part in other activities which doesn't contradict with Caucasian countries legislation and their International obligations.
    9.2. The Union will cooperate with the International Agency of Cancer Research (IARC, Lyon), American Cancer Society (ACS, New-York), Oncology Centers and dispensaries of Caucasian countries, National Cancer Institute (NCI, Bethesda), Cancer Research Foundation of America (CRFA), other international oncology centers and Center of Disease Control (CDC, Atlanta).
    9.3. The Union will be integrated and cooperated with the following international structures:
  • To integrate Union with International Union Against Cancer (UICC, Geneva);
  • To integrate Caucasian Network of Cancer Registries (CNCR) with European Network of Cancer Registries (ENCR, Lyon);
  • To integrate Caucasian Tele-medical Board (CTC) with International Tele-medical Boards;
    9.4. UCCC-Tbilisi may become representative of International Union Against Cancer (UICC) in Caucasus.

    Paragraph 10. UCCC REORGANIZATION AND LIQUIDATION

    10.1. The Union may cease existence on a basis of reorganization (merging, unification, division) or liquidation.
    10.2. Reorganization and liquidation of the Union may be decided:
  • By the Union Members Joint Meeting and requires a two-thirds majority;
  • By competent state organs on the basis of systematic violation of the state legislation from the Union side.
    10.3. Liquidation of the Union is carried out according to the following principle:
  • The Union congress forms Liquidation Commission, which will hold Union funds and property;
  • Liquidation Commission divides funds and property of the Union according to the legislation currently in force.
    10.4. The President of the Union looses his authority at a moment of formation of Liquidation Commission.
    10.5. The Union looses the status of juridical structure in the corresponding state organ at a moment of registration of its liquidation.
    10.6. Information on the Union liquidation will be spread by mass media.
